The updated requirements for entrance into Kindergarten:
- 4 doses of DTaP, DTP, DT pediatric
- 3 doses of Polio
- 3 doses if Hepatitis B
- 2 doses of MMR
- 2 doses of Varicella
- 1 dose of Hep A
Grades 1st thru 12th are as follows:
- No fewer than 3 doses of DTAP, DTP, DT (pediatric), TD- adult, Tdap-1 dose of Tdap for students 11 and older.
- 3 doses of polio with one dose after the age of 4
- 2 doses of MMR
- 2 doses of varicella
- 3 doses of Hepatitis B
- Meningococcal before 7th grade and at age 16. Only one dose if the first one was at age 16 or older.
- 1 dose of Hep A
