Return for 2nd dose or get your 1st in this Covid-19 vaccination clinic Aug 11th in Bryant

Students returning for their second Pfizer vaccine dose (1st dose administered on Wednesday, July 21) should bring their VACCINATION CARDS to the clinic, along with completed COVID-19 Vaccination and FERPA consent forms.

A parent must attend with any student under 18 years of age. The Pfizer vaccine will be administered with no out of pocket expenses.

To receive the COVID Vaccine, students must bring the following:

1)  Completed form by parent/guardian:

 Bring completed forms the day of the clinic. Pre-registration is not required.

Health considerations before your child receives the voluntary COVID-19 Vaccine:

• Your student must be 12 years of age on or before August 11, 2021.
• If your child has had a severe allergic reaction to any vaccine or injectable therapy (intramuscular, intravenous or subcutaneous), your child should not receive the COVID-19 vaccine at this time.
• If your child received monoclonal antibodies or convalescent plasma as part of COVID-19 treatment, your child should defer the vaccine for at least 90 days after treatment.
• If your child is immunocompromised, please talk to your healthcare provider before receiving the COVID-19 vaccine.
• If your child is pregnant, suspected to be pregnant or breastfeeding, please talk with your child’s pregnancy healthcare provider to get a letter of approval for receiving the COVID-19 vaccine.
