The Arkansas Department of Health (ADH) is reminding parents of how to get immunizations or a copy of a birth certificate for sending children back to school.
Parents can go to the local health unit to get the required immunizations. In Saline County, the address is 1612 Edison Ave, Benton, AR 72015. The phone number is (501) 303-5650. To obtain an immunization record, visit www.healthy.arkansas.gov/images/uploads/pdf/Shot_Record_Release1.pdf or contact [email protected] or 501-661-2169. See more information about immunizations for children at www.healthy.arkansas.gov/programs-services/topics/immunizations.
There are a variety of ways to obtain a birth certificate through the ADH. You can request a record in person at a Local Health Unit in Benton, or the Little Rock office at 4815 W. Markham Street, for same-day pick up in most cases. You can also order a record through the ADH website; mail a form available online or at your local health unit; or call 866-209-9482. Birth certificates may be ordered by the parents as listed on the certificate, maternal grandparents, legal guardian or the individual named on the birth certificate if the child is 16 or older. A valid photo identification card must be provided to obtain a birth certificate. This ID must contain a photo, current address, signature, and a unique number or barcode assigned to the person. This could include a driver’s license or ID card, a passport, a visa and more. The first copy costs $12 and each additional copy costs $10.
